Prevent T2: Proven Program to Prevent or Delay Diabetes (TB)

Prevent T2 is a free year-long CDC recognized program by the National Diabetes Prevention Program to help individuals delay or lower their risk of developing Type 2 Diabetes. Take the risk assessment linked below to qualify. This program is covered by most insurances with no out-of-pocket costs. If you do not have insurance, you may be eligible to receive assistance for services at no or low cost to you. Participants will not be excluded due to inability to pay. How do I know if I qualify?
