Routine vaccines available for anyone age 3 or older, COVID-19 vaccines for age 3 and older. Please bring your insurance card for billing purposes. Register using the QR code and select the clinic you wish to attend or by visiting https://patients.vaxcare.com/registration and using enrollment code IN97382. As part of the Wellness Rewards program, eligible employees can earn $25 per vaccine, up to $100. Note: To earn the Wellness Reward incentive, you must provide insurance information when registering and must be billed through insurance.
